Dental Insurance
There are many dental benefit policies out there and not all are created equal. In order to make your first visit more efficient, we would appreciate having the following information about you plan prior to your appointment:
- Name of Insurance Company
- Policy / Group Contract Number
- Identification / Certificate Number
- Benefit Year Starting Date
- Benefit Maximum
If you do not have the answers to these questions, you can look in the booklet provided by your employer, phone your dental insurer or go online to your providers website.
If you are covered under more than one plan, please bring all the information related to all plans. We will be happy to sort out all the details.
If you are covered under your spouse, common-law or parents dental plan, we will need the full name and date of birth of the primary insurance holder.
Please supply this information for all plans under which you are covered and include the name and date of birth of the insured. We endeavour to check your coverage by sending pre authorizations.
If the insurance company responds to you directly, please forward the information to us for your file. info@designdentistry.ca

What does “basic” and “major” coverage mean?
Most dental procedures fall into the “Basic” category (eg. hygiene therapy, dental x-rays, dental exams, fillings, root canals, dentures etc.) Few procedures fall into the “Major” category such as crowns, bridges and implants etc. Most plans ask for a predetermination for “Major” work. This ensures that you know exactly what will be covered by your plan.
